A bug has been discovered in our 6.4.0 release that was preventing our upgrade system from deleting old sign up files that are conflicting with the new client UI template.
If you have upgraded from a previous version and are experiencing issues with the sign up/order pages not displaying properly, please delete the following files:
Once these files are deleted it should resolve the issue.
